Families must meet income guidelines. (Families that receive SNAP, TAFDC benefits, are on specific MassHealth Plans (e.g. MassHealth standard) or are applying for a foster child are automatically income eligible.) If you don't fall under one of these categories, you must provide proof of income . Please call 617-575-5370 for more information. Walk-ins are welcome. Please bring the following to your appointment: 1) proof of income ( or proof of participation in one of the programs listed above) 2) proof of identification 3) proof of residency (your address) 4) the person who is applying for the program
