The Ages and Stages Questionnaire is available to pick up at the Center for Families office, Monday-Friday. Please call ahead to let us know the age of your child and when you are coming so that we can have it ready for you to take home. This questionnaire can be done by parents in their own homes or Center staff are available to help parents do the Questionnaire either at the family's home, in the Center's play space, or another convenient location. Center staff are available to help families understand the Questionnaire and no Questionnaire answers are shared. Everything is confidential. Christine Doucet is available for free to come to your home or meet somewhere in the community to do the ASQ with you and your child in about 20 minutes. You can also pick up the questionnaire to do on your own!
